The message “There was an issue creating your form.” is one of the most annoying messages a WordPress developer can receive. But don’t worry! Some common fixes usually work for this problem.

There was an issue creating your form. Gravity Forms
There was an issue creating your form

Error Status: Error Migrating Entry Headers: Table 'local.wp_rg_lead' doesn't exist NAN% complete. Force the upgrade

database error

Try the guide below if you have clicked on the Force the upgrade button, and it does not resolve the issue.

How to Fix the Gravity Form Error:

  1. [Optional] Export your forms and entries (Forms >> Import/Export)
  2. Uninstall Gravity Forms via the plugin General Settings page (Settings > Uninstall)
  3. Install the plugin again via the plugins page. (Plugins >> Add new)

You can uninstall Gravity Forms without any negative consequences to your contact form. When you reinstall it again after doing so, all your tables will still be there and ready to go! Gravity Forms does not drop tables on plugin deactivation or deletion via the plugins page, so exporting a backup of the forms isn’t necessary, but it’s a reasonable precaution.

Gravity Forms will only drop tables after uninstalling via the plugin settings, at which point GF will create the required tables again upon subsequent activation or installation.

After adding Gravity Forms again, you should have resolved the issue. Please let me know if it didn’t work, and I can help troubleshoot further.
